Service Request Form The Service Request staff provides administrative support for processing requests for information technology. You may contact the Service Request group at (410) 516-0110 or email at srsupport@lists.johnshopkins.edu . We are staffed Monday through Friday from 8:00 am to 5:00 pm. Should you call and receive voicemail, leave your name, a telephone number where you can be reached, and reason for calling. Voicemail and email are checked periodically throughout the day. For ALL customers: The form is ONLY for data connection - not for voice or fax. For EB customers (including Mt. Washington and all JHH/JHHS locations): If wiring is not required, please call the Help Desk (5-HELP) and request that your jack be activated. For EB customers ((including Mt. Washington and all JHH/JHHS locations): If additional wiring is required, please go to itsr.jhmi.edu and complete a Service request OR call 410-516-0110 and ask for help with your request. NOTE: This form is only for jack activations on the University campus at this time.
